In Home Support
Case managers collaborate with seniors and support their families and caregivers, helping them to identify and take advantage of programs and community resources that support their well being and independence. They can meet with seniors in their own homes. All assistance is confidential and under the direction of the individual.
Support for Long Term Care
The Choices for Care Program provides services at home and in the community to individuals who would otherwise need nursing home care and supervision. Our case managers (as well as Home Health Agency case managers) are certified to provide case management under the program.
